This is a statutory requirement to assess the extent/offer of pharmacy services in Gateshead and is used as the market entry document when assessing potential new pharmacy services. It is therefore very important that the council have an accurate picture of current services.
With that in mind, please can you complete the survey for pharmacies. The survey asks for information on subjects including opening hours, consultation facilities, appliances, and services provided.
The council have decided to use their own in house platform to gather this information. The link below will take you to their site. They have incorporated a ‘Save now to continue later’ button should you wish to fill parts in and continue later. If you wish to use this feature click the button and you will receive a code or ID (please be sure to record this in a safe place) that you can use to continue completing the survey at a later time. You can do this multiple times, but must record and use a new code/ID each time.
The last page includes a ‘Finish’ button which will submit your final response, so please be sure you have completed all questions before submitting.
The deadline for this questionnaire is Thursday 30th September.

Patient / Public Survey
At the same time as the pharmacist survey above, the council are also running a survey for the public from 13th September to 24th October. This is available online at www.gateshead.gov.uk/consultationsnew
The Council would like to ask for your help by putting a handful of paper copies on display in your pharmacy, perhaps on the counter, so that customers can complete a copy while they wait. If possible, please print 10 copies of the survey (it will fit on one sheet of A4 paper if printed double sided) and return any that you receive, either by post or by scanning and emailing, once the survey closes on 24th October.
