The Healthier You: NHS Diabetes Prevention Programme (NDPP) is a joint initiative between NHS England, Public Health England and Diabetes UK. It aims to deliver services at a large scale, for people already identified with non-diabetic hyperglycaemia who are therefore at high risk of developing Type 2 diabetes. High risk individuals will be offered a behavioural intervention to enable them to reduce their risk of developing Type 2 diabetes through weight loss, as a result of improved diet and increased levels of physical activity. The programme launched in South Tyneside in 2018.
From July 2020, the referral model has changed from a GP referral-only model to include an additional self-referral model. The public will be able to use an online tool, hosted by Diabetes UK, to calculate their risk of developing the condition by answering a series of basic questions including age, weight and ethnicity. If their score comes back as moderate or high, they will now be able to refer themselves to a local Healthier You service for support remotely or online, without having to go through a healthcare professional. Those who qualify will be able to choose how they complete the programme, either by joining group sessions by video link or telephone with an experienced coach or through digital support, which includes online peer support groups and in some areas, wearable tech.
